That’s pretty standard in book publishing - the author is almost
always responsible for the index. That’s not been hard on me, since I
spent about 10 years as a book indexer. But most authors find doing
their own index a crushing chore (and crushing bore) - but contacting
out for it costs about $2-3 per book page.
